The important points are to make sure the IOL is right side up to start and get the IOL to fold such that the sides of the optic are up, and back of the optic is down against the cartrige. I like to fold both of the haptics so they are inside the optic like a taco. However it works fine if the leading haptic is not folded in and just lays out. The plunger of the inserter goes along the floor of the cartridge to push the IOL into the eye.
