In the era of intracapsular cataract surgery, an extracapsular approach was used on congenital cataracts - either through needling or through a limbal incision. Refined handling of lost vitreous had not yet been developed. The possible presence of an anterior hyaloid (vitreous) attachment to the posterior capsule of the young lens made intracapsular extraction a procedure fraught with danger. The capsular remnants rimming the inferior pupil margin show the successful results of extracapsular surgery in this young eye. Notice the quality of the lashes - one sign of a youthful patient.
